The Essential Glaucoma Tests You Need to Know About

Glaucoma, often called the “silent thief of sight,” is a progressive eye disease that damages the optic nerve, primarily due to elevated intraocular pressure (IOP). If left untreated, it can lead to irreversible vision loss. While medications and laser treatments help manage glaucoma, some patients require surgical intervention for effective long-term IOP control.

At LJEI Ambala, our expert ophthalmologists specialize in trabeculectomy, a highly effective glaucoma surgery designed to restore fluid drainage and prevent further vision loss.

What is Trabeculectomy?

Trabeculectomy is a microsurgical procedure that creates a new drainage channel for the aqueous humor (the fluid inside the eye). By bypassing the eye’s natural but clogged drainage system (the trabecular meshwork), this surgery helps lower IOP and protect the optic nerve from further damage.

How Does It Work?

  • A small flap is made in the sclera (the white outer layer of the eye).
  • A tiny opening is created beneath this flap to allow excess fluid to drain out.
  • A filtering bleb (a small, fluid-filled blister) forms under the conjunctiva (the clear outer eye layer), acting as a reservoir for controlled drainage.
  • The surgeon carefully adjusts the flap to regulate fluid flow, ensuring optimal pressure reduction.

Who Needs Trabeculectomy?

Trabeculectomy is typically recommended for patients with:

  • Uncontrolled glaucoma despite maximum medical therapy (eye drops, oral medications).
  • Rapidly progressing vision loss due to high IOP.
  • Advanced glaucoma where less invasive treatments (like SLT or MIGS) are ineffective.
  • Certain secondary glaucomas (e.g., uveitic glaucoma, neovascular glaucoma).

However, it may not be suitable for patients with:

  • Severe conjunctival scarring (from previous surgeries or infections).
  • Extremely thin corneas.
  • Active eye infections or inflammation.

The Trabeculectomy Procedure: What to Expect

Before Surgery

  • A comprehensive eye exam will assess your glaucoma severity.
  • You may need to stop certain medications (like blood thinners) before the procedure.
  • Antibiotic eye drops may be prescribed to prevent infection.

During Surgery

  • Anesthesia – Local anesthesia (numbing drops & injections) is used; general anesthesia may be an option for some patients.
  • Creating the Flap – A small, partial-thickness flap is made in the sclera.
  • Removing the Trabecular Meshwork – A tiny piece of the eye’s clogged drainage tissue is removed.
  • Iridectomy (Optional) – A small part of the iris may be removed to improve fluid flow.
  • Bleb Formation – The conjunctiva is repositioned to create a filtering bleb, hidden under the eyelid.
  • Suturing – The scleral flap is stitched loosely to regulate fluid drainage.

Recovery & Post-Operative Care

First Few Days After Surgery

  • A protective eye shield is provided to prevent accidental rubbing.

  • Mild discomfort, redness, and blurred vision are normal.

  • Antibiotic and anti-inflammatory eye drops are prescribed to prevent infection and reduce swelling.

First Few Weeks

  • Avoid heavy lifting, bending, or straining (can increase eye pressure).

  • No swimming or exposure to dusty environments (risk of infection).

  • Attend all follow-up visits to monitor IOP and bleb function.

Long-Term Care

  • The bleb must remain functional for continued success.

  • Some patients may need additional procedures (e.g., bleb needling) if scarring occurs.

  • Lifelong glaucoma monitoring is essential, even if pressure is controlled.
