Explain Optical Penetrating Keratoplasty

Optical penetrating keratoplasty (OPK) or penetrating keratoplasty (PKP) is a surgical procedure involving the complete replacement of the host cornea with a full-thickness donor corneal graft. The use of an appropriate-sized trephine facilitates the removal of the recipient cornea. When performed with diligent postoperative care, corneal transplants can yield successful outcomes lasting for decades. However, OPK carries potential intraoperative and postoperative complications that require timely management to achieve optimal results. Graft rejection episodes can occur at any time following optical keratoplasty, and multiple risk factors contribute to the likelihood of rejection.

If a patient suffers poor vision or chronic pain from a corneal condition, disease, or injury, they are likely a good candidate for a corneal transplant. Because penetrating keratoplasty involves a complete transplant of the cornea, it is often the best treatment option for those with more severe corneal disease.

Optical Penetrating Keratoplasty In Details

Corneal diseases are one of the major causes of
blindness and visual impairment in developing
countries. Corneal transplantation, also known as
corneal grafting or penetrating keratoplasty, is a surgical
procedure where a damaged or diseased cornea is
replaced by donated corneal tissue which has been
removed from a recently deceased individual having
no known diseases which might affect the viability of
the donated tissue.

Although newer modalities of keratoplasty have come
into play but penetrating keratoplasty (PK) still remains
the gold standard in corneal conditions involving full
thickness of the cornea. Corneal transplantation has
two major types, penetrating keratoplasty (PK) and
lamellar keratoplasty (LK). The term penetrating
keratoplasty (PK) commonly refers to surgical
replacement of a portion of the cornea with that of a
donor eye. Lamellar keratoplasty surgery consists of placing a partial thickness donor corneal graft in a recipient corneal bed that is prepared by lamellar dissection of diseased anterior stoma corneal tissue.

What are the indications for optical keratoplasty?
The main indications for keratoplasty are to improve vision (optical), preserve corneal integrity (tectonic), or remove infected cornea (therapeutic). The preoperative evaluation and preparation is outlined. The surgical procedure involves donor and host cornea trephination and suturing of the donor button.
What are the indications for PK?
The specific indications for PK are varied and can include corneal dystrophies and degenerations, congenital opacities, bullous keratopathy (BK), trauma, keratitis, and immune rejection and/or failure of a previous corneal transplant
What is the vision after penetrating keratoplasty?
Postoperative astigmatism has been reported in the range of 2 D to 8 D , and spherical equivalents in the range of 2D to 12 D. In one examination of post-corneal transplant patients, less than half (48%, 21 of 43 eyes) achieved refractive error within 2 D of emmetropia.

What Is A Lamellar Keratoplasty?
A Lamellar Keratoplasty is also known as a partial-thickness corneal transplant. Unlike a full-thickness corneal transplant, this procedure only requires a thin layer of the cornea to be removed. Individuals who have surface-level corneal scars or corneal scarring that is not deep may get this procedure. Individuals with keratinous are also candidates for partial-thickness corneal transplants.
What Is An Endothelial Keratoplasty?
The endothelial Keratoplasty is one of the most commonly received corneal transplants. This procedure involves the thinnest layer of corneal tissue needed to be removed and replaced. This procedure requires the least amount of healing time but requires the most skill. The thinness of the layer makes the tissue difficult to work with.

Fungal Keratitis

bullous keratopathy, keratoconus, dystrophies, degenerations and scarring.

Tectonic grafting may be carried out to restore or preserve corneal integrity in eyes with severe structural changes such as stromal thinning and descemetocele.

Therapeutic corneal transplantation may afford removal of infected corneal tissue in eyes unresponsive to antimicrobial therapy.

Cosmetic grafting may rarely be performed to improve the appearance of eye.
