What is DSEK surgery and who need it?

Descemet’s Stripping Endothelial Keratoplasty (DSEK) is a procedure that involves replacing only the innermost layer of the cornea, rather than the entire thickness of the cornea, as is performed in standard full-thickness corneal transplantation. For patients who have conditions such as Fuchs corneal dystrophy or swelling of the cornea after cataract surgery, corneal clouding is caused by dysfunction of the cells that make up the innermost layer of the cornea, called the corneal endothelium. The corneal endothelial cells function to pump fluid out of the cornea. When these cells are damaged or reduced in number, the cornea swells, resulting in corneal clouding and loss of vision. Even though the other layers of the cornea are healthy, this condition has traditionally been treated with full-thickness corneal transplantation to bring a new layer of endothelial cells to the patient’s eye.

DSEK surgery involves peeling only the diseased endothelial layer from the back of the patient’s cornea, leaving the remaining 95% of the cornea, which is healthy, untouched. The posterior layer of a healthy donor cornea is then placed inside the eye through a small incision and positioned with an air bubble to replace the diseased layer that was removed. DSEK is performed in less than one hour without any sutures in the cornea.

DALK can be performed in Corneal ectatic disorders like Keratoconus, Post LASIK Ectasia, Pellucid marginal corneal degeneration (PMCD). The bulged out cornea (forward bending making it a conical shape) in these disorders makes the focus in patient eye distorted and blurry. In this surgery, only front layers of cornea (stroma) is first cut with advanced corneal trephines and later dissected using special techniques and instruments. After this, same layers from a donated cornea is procured and then transplanted over patient’s eye. The ectatic or bulged out cornea is hence replaced by healthy cornea with appropriate shape. Use of Femtosecond laser in this surgery helps in avoiding use of trephine and various blades making it very accurate with diameter and depth of corneal layer transplantation. There is almost negligible risk of graft rejection and failure in this technique of cornea transplantation (Keratoplasty).

What to expect after DSEK surgery?
Your eye will take 6-8 weeks to heal. It is normal for your eye to be red, slightly uncomfortable, light sensitive, teary and blurred immediately following surgery. These symptoms will gradually improve within the first 3 weeks.
What are the restrictions after a partial corneal transplant?
Do not wear eye makeup until your doctor says it’s okay. You may also want to avoid face cream or lotion. Avoid heavy lifting for about 4 weeks, or until your doctor says it is okay. Avoid swimming, hot tubs, gardening, and dusting for 2 to 4 weeks.
How long does DSEK last?
A DSEK procedure is done with the patient under local anesthesia. The procedure takes just 45 minutes, about half the time needed for a traditional cornea transplant.
How successful is DSEK surgery?
At 10 years, DSEK survival rate was 79% for all eyes, including complex grafts, and ECL was 73%.
Can I watch TV after a cornea transplant?
Refrain from any eye makeup for 7 days following surgery. Avoid strenuous activity. You may read and watch TV as much as you would like. You may resume work and driving when you feel ready.
What is the recovery time for cornea surgery?
during the first weeks after surgery, avoid strenuous exercise and heavy lifting. if you have a job that does not involve physical strain, you can return to work 2 to 3 weeks after surgery. if your job involves manual labour, you should wait for 3 to 4 months.
What is the vision after DSEK?
By three months, the vision is usually around 20/50 or better, but this is highly variable, with many patients achieving 20/30 or so. At one year after surgery, over 65% of patients have 20/40 or better vision.
What causes failure of DSAEK?
The principal cause of failed DSAEK is endothelial cell loss. Residual host Descemet membrane, fibrocellular tissue at the edge of the lenticule, and epithelial implantation are common histologic findings. Fungal infection may occur in the setting of DSAEK.
Which is better DSEK or DMEK?
In clinical trials, DMEK has been shown to offer patients a slightly better quality of vision and to resume their daily activities quickly. There is also a slightly lower long-term risk of graft rejection with DMEK compared to DSEK.
What is the rejection rate of DSEK?
the average incidence of primary graft failure in Descemet stripping endothelial keratoplasty (DSEK) is 5%, and the incidence of mean endothelial rejection rate is 10%.
Can you have 20/20 vision after a cornea transplant?
A vision of 20/20 is possible, but even with a crystal-clear cornea, vision depends upon the function of the retina and may not attain 20/20. It is most important to remember that while the vision will be better than it was before surgery, vision improvement varies among all patients.
Can I travel after corneal transplant?
Following DMEK and DSAEK, patients may not fly or drive to an elevation greater than 2,000 ft to avoid elevated pressure caused by the bubble used to attach the transplant tissue. The bubble is generally only big enough to limit travel for 1 day following DSAEK and for 4-5 days following DMEK.
Can cornea be replaced twice?
Corneal transplants can be re-transplanted if they fail, scar, or produce poor vision. Transplants can be repeated multiple times as long as there is a reasonable chance of success and the eye and patient can tolerate additional procedures.
