What is Keratoprosthesis?

Keratoprosthesis is a surgical procedure where a diseased cornea is replaced with an artificial cornea. Traditionally. Keratoprosthesis is recommended after a person has had a failure of one or more donor corneal transplants. While conventional cornea transplant uses donor tissue for transplant, an artificial cornea is used in the Keratoprosthesis procedure. The surgery is performed to restore vision in patients suffering from severely damaged cornea due to congenital (birth) defect, infections, injuries and burns. Keratoprothesis is made of clear plastic with excellent tissue tolerance and optical properties. The KPro has been used include multiple graft failure, Stevens-Johnson syndrome (SJS), ocular cicatriclal pemphigoid (OCP), other autoimmune diseases, ocular burns (acid and alkali) and other conditions with poor prognosis of traditional corneal transplant.

Who need Artificial Cornea?

The success rate of corneal transplants has increased markedly over the last 40 years, but there are some situations where human donor transplants don’t work. An artificial cornea fills this void. The device is a huge step forward for people with corneal blindness who have rejected human tissue or who live in areas where corneal tissue is not readily available.

Which layer of cornea Cannot regenerate?

Bowman’s membrane lies just anterior to stroma and is not a true membrane. It is acellular condensate of the most anterior portion of the stroma. This smooth layer helps the cornea maintains its shape. When injured, this layer does not regenerate and may result in a scar.

How many layers of cornea are there?

The human cornea is comprised of six different cell layers: Epithelium, Bowman’s Layer, Stroma, Dua’s Layer, Descemet’s Membrane and Endothelium. The epithelium is the outermost layer of the cornea and accounts for about 10% of the cornea tissue’s thickness.

My Approach

1. Whether the patient really need cornea transplant or not. Although cornea transplant is most successful transplant in human body. Still transplant is transplant. If not that much necessary we will not do transplant.

2. If it really needed then we will see which technique is most appropriate for that patient. It is not a blanket that every cornea patient need same technique. So cornea transplant technique very from one patient to another. We see what is disease, type of transplant needed which technique will give highest success rate like 90% to 95%.

3. Follow Up : we have developed a strong follow up system. We take regular follow ups. So that rejection chances or infection chances are minimum.

I think how in compassionate manner I can treat this patients. I think like this is my family member and how I can treat my family member in best possible way.

Keratoplasty Failure?

Corneal diseases are a leading cause of blindness with an estimated 10 million patients diagnosed with bilateral corneal blindness worldwide. Corneal transplantation is highly successful in low-risk patients with corneal blindness but often fails those with high-risk indications such as recurrent or chronic inflammatory disorders, history of glaucoma and herpetic infections, and those with neovascularisation of the host bed. Moreover, the need for donor corneas greatly exceeds the supply, especially in disadvantaged countries. Therefore, artificial and bio-mimetic corneas have been investigated for patients with indications that result in keratoplasty failure.
